Program Summary: South Burnett Carer Support

South Burnett Carer Support is a practical support pilot for foster and kinship carers. The program provides assistance at times and in ways that respond to carers’ identified needs, working alongside existing services to strengthen placement stability and improve outcomes for children and young people in care.

Support may include practical in-home help, transport and supervision, routines and appointment support, school engagement and community participation, mentoring/role modelling, assistance for carers to attend training or care team meetings, and approved short-term or one-off brokerage (where appropriate and within program specifications).

Position Overview

The Carer Support Practitioner combines program coordination with direct practitioner responsibilities. The role coordinates referrals, prioritisation, service planning and day-to-day delivery while also working directly with foster and kinship carer households through short to medium term, fixed engagements.

For each accepted engagement, the Carer Support Practitioner develops and leads a carer-led Single Case Plan and coordinates the right people, services, practical supports and approved brokerage to achieve clearly defined outcomes. The practitioner acts as a central coordination point for the household, working with carers, children and young people where appropriate, Child Safety, schools, health and community services, and youth workers to avoid duplication and ensure supports are timely and purposeful.

The Carer Support Practitioner also oversees key operational elements of the pilot, including referral flow, allocation and briefing of rostered Youth Support Workers, service scheduling, case coordination, brokerage administration, recording and reporting. The role identifies emerging demand and potential service gaps to support continuous program improvement.
