Our services encompass areas such as Foster Care, Residential Care, Youth & Family Services, Domestic and Family Violence Services, and Disability Services, operating throughout the South Burnett region from our offices in Kingaroy, Wondai, Nanango, and Murgon.

Position Overview

The Transitions Coordinator will play a crucial role in providing tailored support to young individuals aged 15-17 in Supported Independent Living (SIL) environments. The successful candidate will collaborate with Child Safety Officers and other staff members to create and implement Transition to Adulthood plans, ensuring that each young person achieves their goals and prepares for a successful future.

This position is integral to the leadership team, participating in an on-call roster and directly supporting young people in SIL houses. The role requires a blend of hands-on support and strategic planning, making it ideal for an experienced youth worker.

Ideal Candidate Profile

The successful candidate will possess a strong background in youth work, with the following qualifications and attributes:

  • Minimum Certificate IV in Child, Youth and Family Intervention, Social Work, or a relevant field
  • Proven experience in Residential Care or related sectors with a focus on engaging and supporting clients
  • Specialised training in trauma-informed care
  • Queensland Open Driver’s Licence and ability to obtain First Aid, CPR, Working with Children (Blue Card), NDIS Worker Screening Check, and Bronze Medallion Certificates
  • Strong time management and IT skills to maintain accurate client records and meet deadlines

The Transitions Coordinator must be flexible and available for out-of-hours on-call responsibilities, adjusting hours as needed to meet client requirements. Cultural sensitivity and the ability to work collaboratively with stakeholders are essential for this role.
