Your Role as a Support Coordinator

As a Support Coordinator at CTC Disability Services, you will work independently to drive support coordination services and assist individuals with disabilities in achieving their NDIS Plan goals. Collaborating with participants and stakeholders, you will connect them with relevant support services, foster independence, and ensure the effective coordination of their supports.

If you have industry experience in Support Coordination or related fields, a commitment to empowering individuals with disabilities, and the ability to work autonomously, you might be the perfect fit for our team. We seek individuals who can engage with participants, build rapport, and navigate the NDIS landscape with compassion and expertise.

  • Demonstrated experience in Support Coordination or similar roles
  • Passion for promoting the rights and independence of individuals with disabilities
  • Ability to work independently and drive support services effectively

Salary & Benefits
